What to Expect 72 Hours After Wisdom Tooth Extraction

Knowing what to expect post-surgery is crucial, especially when it comes to wisdom tooth extraction. At Precision Dentistry of Howard, Dr. Craig O’Donoghue ensures patients receive top-tier care. With an educational background from the University of Iowa College of Dentistry and extensive training in IV sedation dentistry, Dr. O’Donoghue prioritizes patient comfort. This blog outlines what you can expect 72 hours after your wisdom tooth extraction, focusing on IV sedation and its benefits.

The Role of Oral Sedation in Dentistry

IV sedation is an advanced method for ensuring patient comfort during dental procedures. This technique involves delivering sedative drugs directly into the bloodstream via an intravenous line, inducing a profound state of relaxation.

Here’s a detailed breakdown of the types of sedation used in dentistry and their effects:

Minimal Sedation:

  • Patients remain awake but are relaxed.
  • Standard methods are oral medications like diazepam (Valium) and nitrous oxide (laughing gas).

Moderate Sedation (Conscious Sedation):

  • Patients are awake but might not remember the procedure.
  • They can respond to verbal commands.
  • This can be achieved through oral sedatives, IV sedation, or a combination.

Deep Sedation:

  • Patients are on the edge of consciousness but can be awakened if necessary.
  • They are unlikely to remember the procedure.
  • This level of sedation is often achieved through IV sedation.

General Anesthesia:

  • Patients are completely unconscious and will not be aware of or remember the procedure.
  • This is typically used for more complex or lengthy dental procedures.
  • General anesthesia is administered by an anesthesiologist or a specially trained dentist using IV medications (like propofol).

What Drug is Used for IV Sedation in Dentistry

The choice of sedative medication depends on the patient’s needs, health history, and the complexity of the procedure. However, these are the most common:

1. Benzodiazepines: Medications like midazolam are commonly used for calming effects.

2. Propofol: Known for its quick onset and short duration, it is ideal for deep sedation procedures.

3. Nitrous Oxide: Nitrous oxide can be combined with IV sedation in dentistry to enhance relaxation.

After Wisdom Tooth Extraction: First 24 Hours

Immediately after your wisdom tooth extraction, it’s normal to feel groggy and disoriented due to the effects of IV sedation. Here’s what you can expect and how to manage the initial recovery period:

1. Monitoring and Rest: After the procedure, you will be monitored until the effects of IV sedation begin to wear off. It’s essential to have a responsible adult accompany you home, as driving is not safe post-sedation.

2. Pain Management: Mild to moderate pain and swelling are common. Your dentist will prescribe pain medication to help manage discomfort, and over-the-counter pain relievers like ibuprofen can also be effective.

3. Bleeding: Some bleeding is expected. Bite gently on gauze pads placed over the extraction sites and change them as instructed by your dentist.

4. Diet: Stick to soft foods like broth, applesauce, and smoothies (no straw), and avoid hot, spicy, or chewy foods. Hydrate with water, but avoid using straws as the suction can dislodge blood clots and hinder healing.

24 to 48 Hours: Reducing Swelling and Managing Pain

As you move into the second-day post-extraction, swelling and pain may peak. Here’s how to manage these symptoms:

1. Cold Compress: Gently apply ice packs to the outside of your cheeks to reduce swelling. Leave the cold compress on for 20 minutes, then 20 minutes off.

2. Medication Adherence: Continue taking prescribed pain medication and any antibiotics as directed to prevent infection and manage pain.

3. Oral Hygiene: Gently rinse your mouth with warm salt water to clean the extraction sites. Avoid vigorous rinsing or spitting to prevent dislodging the blood clots.

4. Diet: Continue with softer foods like mashed potatoes, scrambled eggs, and pudding.

48 to 72 Hours: Continued Recovery and Observation

By the third day, you should start feeling a bit better, but it’s essential to continue proper aftercare:

1. Warm Compress: Switch to warm compresses if you experience any lingering swelling.

2. Diet and Hydration: Gradually reintroduce more solid foods like soft pasta, oatmeal, and soft-cooked vegetables into your diet as tolerated. Continue to avoid foods that can irritate the extraction sites.

3. Activity Level: Rest is crucial, but light activities can resume as you feel comfortable. Avoid strenuous exercise as it can increase bleeding and swelling.

IV Sedation: What Makes It Effective?

IV sedation is favored in dentistry for several reasons:

  • Rapid Onset: The sedative effects begin quickly, allowing for efficient and timely procedures.
  • Customizable Dosage: Dosages can be adjusted during the procedure to maintain the desired level of sedation, ensuring a tailored experience for each patient.
  • Maintained Consciousness: Depending on the procedure and the desired level of sedation, patients can remain conscious and responsive despite deep relaxation, enhancing cooperation and safety during the procedure.
  • Enhanced Comfort: IV sedation significantly reduces anxiety and discomfort, making it ideal for patients with dental phobias or those undergoing extensive dental work.

My experience with Precision Dentistry has been stellar and a blessing. If you are looking for a one-stop shop (extractions, implants, full mouth reconstruction, etc.) as well as very personalized and accommodating care, then Precision Dentistry should be your choice. Unlike the corporate run, impersonal dental offices that I have experienced, Precision Dentistry provides expert and patient-focused care. Dr. O'Donoghue listens to your needs/issues, carefully explains his diagnosis and options in an easily understandable fashion, and works diligently to deliver the final result in the desired timeframe. After being warned by multiple dentists for years that my teeth were very worn and that I would start experiencing cracked teeth, root canals/crowns, implants, etc., I could choose to address these problems as they arose or do a full-mouth reconstruction. Having yet another cracked, abscessed tooth, I visited Dr. O'Donoghue for advice based upon a referral from another Green Bay dentist - it should be noted that I reside part time on an Island 3 hours from Green Bay. During this initial meeting in mid-July, I told Dr. O'Donoghue that I'd been waiting for 3 weeks for Door County Oral Surgeons to schedule an appointment to extract the troublesome tooth. He took an X-ray and advised that he could extract the tooth that day rather than having me wait for an Oral Surgeon appointment. I said: let's do it. The tooth was successfully extracted; he also did a bone graft to prep for an implant. He then reviewed his 20+ years of experience in full mouth reconstructions, advised that he preferred the durability and consistent look of zirconium crowns (versus a mix and match of crowns and veneers), and outlined his background in working with and accommodating patients who reside hours away from his office (he practiced in Alaska for a couple of decades, where patients traveled great distances). I also advised that I planned to leave the Island for my permanent residence in California at the end of October and asked if there was enough time to do the full-mouth reconstruction before the end of October (every other dentist that I spoke to in Door County and Green Bay said: no). Dr. O'Donoghue said: yes. Dr. O'Donoghue also reviewed crown pricing, which is $1000 less per tooth than the price that my California dental office quoted to me a year ago. In this regard, my California dental office transitioned from a family-style dentist to a corporate-style office a decade or so ago and has a revolving door of new, recently graduated dentists. Besides the cost, this is the other reason why I'd been hesitant to proceed with a full-mouth reconstruction with my California dental office. Precision Dental's boutique-style of expert service and reasonable pricing eliminated my concerns in proceeding. It is now the beginning of October and I have 27 zirconium crowns and the implant prepped for the tooth that was extracted in mid-July - Dr. O'Donoghue will make and install the implant (28th crown) when I return to Wisconsin in May 2024. I'm closing in on my 60th birthday and I must say that this is the best medical/dental care that I have experienced in my lifetime. In this regard, not only is Dr. O'Donoghue a gem, but so are his assistants (Katerina and Sophia). Thank you Precision Dentistry!

Absolutely fabulous implant consultation. Both Katrina and Dr. Craig were extremely thorough. They walked us through the entire implant process. Showing us the differences, explaining how each one functions as well as walked us through step by step how the process would begin. Highly recommend.

I want to thank everyone for the best pain free tooth extraction I've ever experienced. Doc was right when he commented the worst it tastes the better it works. 😎 Even after the numbing wore off I (my nosey tongue) had to check and see if it was really gone because there was no pain. I'm not complaining !! Thanks again, Pam
